National Physical Therapy Examination — Physical Therapist
The NPTE for Physical Therapists is developed and administered by FSBPT and is used by U.S. licensing jurisdictions to assess entry-level competence. Passing the NPTE is a major licensure requirement, but candidates must also satisfy the specific requirements of the state or jurisdiction where they seek a physical therapist license.
Format: Computer-based multiple-choice examination administered at Prometric test centers.
Total Questions: 225 questions across 5 sections of 45 questions each.
Scored / Unscored: 180 scored items and 45 unscored pretest items. The unscored items are mixed throughout the examination and are not identified to candidates.
Testing Time: 5 hours.
Total Appointment Time: 5 hours 30 minutes, including the scheduled break, pre-exam tutorial, and post-exam survey.
Scheduled Break: 15 minutes after Section 2. Optional breaks after other sections use exam time.
Question Types: Stand-alone multiple-choice items, including questions with graphics or video, plus shared clinical scenarios. Scenario-based items appear at the end of each section, with 2–5 independent questions linked to each scenario.
Passing Standard: Scaled score of 600 on FSBPT's 200–800 reporting scale. The NPTE is pass/fail; 600 or higher is passing.

The current FSBPT PT content outline allocates 180 scored items across body-system and nonsystem content. Ranges below are official FSBPT ranges for scored questions; the full 225-question exam also contains 45 unscored pretest items.

🗺️ U.S. Physical Therapist Licensure Pathway — NPTE / FSBPT
FSBPT administers the NPTE, while individual U.S. jurisdictions issue physical therapist licenses. Requirements and sequencing vary by jurisdiction.
Choose your licensing jurisdiction
Review the PT licensure requirements of the state or jurisdiction where you intend to practice. Some jurisdictions may require additional items such as a jurisprudence examination.
Complete or initiate the jurisdiction's licensure/eligibility process
Submit the application, education records, and supporting documentation required by your licensing authority. Requirements and sequencing vary by jurisdiction.
Register and pay for the NPTE through FSBPT
Register for the NPTE through your FSBPT account and pay the current examination fee. Request testing accommodations during registration if applicable.
Obtain approval to test
Your jurisdiction or authorized approval pathway must make you eligible. FSBPT sends the Authorization to Test (ATT) only after registration/payment and eligibility approval are complete.
Schedule the NPTE with Prometric
Use the instructions in your ATT to schedule the examination at Prometric. Schedule promptly because testing-center seats can fill.
Complete the NPTE-Physical Therapist Exam
Sit for the 225-question NPTE-PT. Testing time is 5 hours across five 45-question sections. A scaled score of 600 or higher is passing.
Receive the examination result
FSBPT reports scores according to the published score-reporting schedule. Your licensing jurisdiction also receives the result.
Complete remaining jurisdiction requirements and obtain your PT license
Complete any remaining state-specific requirements. The licensing jurisdiction, not FSBPT or Prometric, issues the physical therapist license.

- 2Practice scenario-based clinical reasoning, not only recall. FSBPT includes shared clinical scenarios with 2–5 independent questions linked to each scenario.
- 3Train for endurance with full 225-question, five-hour simulations and practice pacing across five 45-question sections.
- 4Give strong attention to safety, red flags, differential diagnosis, referral, prognosis, intervention selection, and evidence-based practice.
- 5Use the official FSBPT content outline to distribute study time and compare your performance by body system and nonsystem area.

- Official Passing Standard: Scaled score of 600 or higher. FSBPT does not use a fixed published raw-score percentage because the raw number correct needed can vary by exam form.
- Practice Goal: Target consistently strong performance across all blueprint domains and complete full-length mocks under official timing.
- Question Style: NPTE-style clinical and knowledge-based MCQs, including shared clinical scenarios and safety-focused clinical reasoning.
LICENSURE CHECKPOINT: Passing the NPTE does not by itself issue a PT license. You must also satisfy the licensing requirements of the U.S. state or jurisdiction where you intend to practice.
IMPORTANT: FSBPT limits candidates to no more than three consecutive NPTE attempts in a 12-month period and no more than six lifetime attempts at an exam level. A candidate cannot continue testing after two scores of 400 or below. After three consecutive attempts, the candidate must skip the next scheduled NPTE administration. Jurisdictions may impose additional restrictions.
The NPTE-PT is administered on fixed dates four times each year, generally in January, April, July, and October. Registration/payment deadlines, jurisdiction-approval deadlines, seat-reservation dates, and score-reporting dates change with each administration.
